If you are a member of an insurance policy of which we are providers you will be responsible for any deductible and/or copayment amounts.

We will file your insurance as a courtesy to you. If you do not have medical insurance, a fair payment plan will be set up at the time of your first office visit.

All balances older than 60 days are considered overdue unless other payment arrangements have been made. Please remember that even If we have filed your insurance claim and will follow up on said claim, the responsibility of payment after the 60 day period is yours.

Any balance due older than 60 days may be turned over to our attorney for collection. If this should be necessary, you will be responsible for all attorney fees, collection fees, interest and court costs. We know that most of our patients will cooperate with the Financial Policy If you have any questions regarding the above, please do not hesitate to ask.
